چکیده

T he approach to the treatment of rheumatoid arthritis (RA) has changed substantially in recent years. Firstly, it is now common practice to introduce conventional disease modifying antirheumatic drugs (DMARDs) such as methotrexate, and even targeted biological therapies, within months of the onset of symptoms. One reason for this change is the growing appreciation that irreversible structural damage can occur very early in the course of inflammatory arthritis. Secondly, the establishment of dedicated early arthritis clinics facilitates early referral of patients with recent onset inflammatory arthritis for specialist evaluation and management. Thirdly, rheumatologists now have access to targeted therapies that greatly reduce the rate of progressive joint damage. Finally, it has been established that DMARDs reduces the rate of progressive joint damage most effectively when introduced within 6 months of symptom onset. In this context, the achievement of a sustained clinical remission in RA, the complete and permanent resolution of synovial inflammation, could become a realistic therapeutic goal. The same should be true of other inflammatory arthropathies.

Should oligoarthritis be reclassified? Ultrasound reveals a high prevalence of subclinical disease.

OBJECTIVE To determine the prevalence of subclinical synovitis using ultrasound (US) imaging of both painful and asymptomatic joints, in patients with early (<12 months), untreated oligoarthritis (</=5 joints). METHODS Eighty patients underwent a detailed clinical assessment by two physicians. All painful joints were identified, which were immediately scanned by a sonographer. In the last 40 ...

Use of Ultrasound for Diagnosis and Follow-up of Psoriatic Arthritis

Musculoskeletal ultrasound (US) is increasingly used as a bedside tool for diagnostic and monitoring purposes in patients with psoriatic arthritis (PsA). The sonographic differentiation between PsA and rheumatoid arthritis (RA) may be challenging because the morphological appearance of synovitis is similar in both conditions. In contrast, perisynovial inflammation is a specific finding of early...

[Sonographic monitoring of gout].

OBJECTIVE To assess the role of grayscale and power Doppler sonography in short-term monitoring of acute gouty synovitis. METHODS Clinical and sonographic assessments were performed in a patient with gout. Clinical and sonographic evaluations were performed within the first day of the acute onset of synovitis of the first metatarsophalangeal joint of the left foot and 7 and 14 days after the ...

Patients with juvenile idiopathic arthritis in clinical remission with positive power Doppler signal in joint ultrasonography have an increased rate of clinical flare: a prospective study

BACKGROUND Ultrasonography (US) studies carried out on joints of juvenile idiopathic arthritis (JIA) patients in clinical remission demonstrate the presence of subclinical synovitis. The significance of subclinical synovitis and the positive power Doppler (PD) signal on US in JIA in clinical remission is not well understood. The objectives of this study were to assess whether the changes detect...

Sonography and joint pressure in synovitis of the adult hip.

Sonography was successfully used to diagnose an effusion in the hips of three adults with septic arthritis and four with aseptic synovitis. The effusions were confirmed by aspiration. All the patients had markedly increased intracapsular pressure which, in the extended position, exceeded the systolic blood pressure and could well compromise the blood supply to the head of femur. Aspiration redu...

Is There Subclinical Synovitis in Early Psoriatic Arthritis? A Clinical Comparison With Gray-Scale and Power Doppler Ultrasound

OBJECTIVE Arthritis activity assessments in psoriatic arthritis (PsA) have traditionally relied on tender and swollen joint counts, but in rheumatoid arthritis, multiple studies have demonstrated subclinical inflammation using modern imaging. The aim of this study was to compare clinical examination and ultrasound (US) findings in an early PsA cohort. METHODS Forty-nine disease-modifying anti...
